A copy of your USA Hockey Registration Form with bar code and confirmation number (Please Note: you must do this electronically on line with USA Hockey).

registration confirmation sheet with bar code and bring it with you to registration.

Requirements for managers and coaches

registration confirmation sheet with bar code.  This will have to be given to the registrar (Jim Eastin) when you are selected to coach a team. EAJHA will reimburse 3 coaches per team.
